Fairfield Glade Clinic

At Cookeville Regional Medical Center, we want to make it easy to get the best care in the region.  That's why we are working to provide services in communities throughout the region.  The Fairfield Glade Clinic of Cookeville Regional is located in the Fairfield Towne Center at 4929 Peavine Road in Crossville near Fairfield Glade.  The clinic will staff a primary care nurse practitioner to care for minor illnesses and injuries, Monday through Friday from 8:00 a.m. to 5:00 p.m.  Family nurse practitioner Elaine Howard, FNP-C, RN, MSN, MHSA has joined the clinic staff to provide those primary care services. 

Cookeville Regional specialists will also provide office hours in the clinic on a regular basis.  Physicians who will be seeing patients at the clinic are as follows:
