Diabetes Center
The Diabetes Center offers an education program focusing on teaching self-management skills to patients with diabetes and their families. The program utilizes a team approach in developing an individualized plan for each participant. The Diabetes Center staff are certified diabetes educators, including registered nurses and a dietitian. Since 1998, our program has been an American Diabetes Association Recognized Program, which means we meet the National Standards for Diabetes Self-Management Education.
The program does require a physician referral. If you are unsure as to whether diabetes self-management is covered by your insurance plan, the Diabetes Center will be glad to check your insurance coverage. The Diabetes Center also hosts and coordinates a diabetes support group, which meets on the second Tuesday of every other month starting in January. The meeting begins at 7pm at the Diabetes Center and is free and open to the public. If you would like more information about a referral to the Diabetes Center, please contact our office at (931) 520-7747.The Diabetes Center is located at 127 North Oak Avenue, approximately two blocks from the hospital.
