Description:

This position oversees and manages the execution of information systems full-life cycle activities for pharmacy and medication-use projects and initiatives.  Participates as a pharmacy and medication use information system expert on pharmacy operational initiatives.  Also, coordinates medication use systems affecting any/all departments when deemed appropriate. Coordinates interdepartmental communication between the pharmacy and other departments with regard to information systems and technology. This position is responsible for pharmaceutical care of patients including all age groups (infants, pediatrics, adolescents, adults and geriatrics).  Also responsible for coordinating Pharmacy Quality Improvement activities involving medication use information systems and teaching pharmacy students and residents as well as participating in community medication education classes..  Uses knowledge and judgment to make decisions concerning patient drug therapy. 

 

  1. EDUCATION: B.S. in Pharmacy or Pharm. D. degree with required internship; completion of a clinical residency program preferred.  Must be licensed by State of Tennessee. 

 

  1. EXPERIENCE:   One-year experience as a pharmacist in a hospital setting preferred.

 

  1. SKILLS:  . Must have thorough knowledge of pharmaceutical drug distribution systems and pharmacotherapy concepts in order to resolve drug therapy related problems; must have ability to communicate effectively with patients and other healthcare professionals.

POSITION SUMMARY:  This position is responsible, under the direct supervision of the pharmacist(s) on duty, for pharmaceutical care of patients including all age groups (infants, pediatrics, adolescents, adults and geriatrics).  This position is also responsible for coordinating Pharmacy Quality Improvement activities as assigned by the Director of Pharmacy or the Pharmacy Manager. The individual in this position is to be alert to innovative ideas that will improve pharmacy services and activities and to share same with the department director/manager.  This position uses knowledge and judgment to make decisions concerning the proper filling of drug orders subject to final approval by the pharmacist(s) on duty. 

  1. EDUCATION:  High school education or equivalent.  Successful completion of certified technician training program with successful completion of PTCB exam or certification by Pharmacy Technician Certification Board.

 

  1. EXPERIENCE:  Pharmacy experience (hospital preferred) or completion of technician training program clinical internships preferred

Description:

  1. EDUCATION: Doctor of Pharmacy from an ACPE accredited program, completion of a PGY1 Pharmacy Residency required. Must be licensed by State of Tennessee. 

 

  1. EXPERIENCE:  Completion of a PGY1 Residency required.

 

  1. ESSENTIAL DUTIES AND RESPONSIBILITIES: The primary job responsibilities include providing comprehensive pharmaceutical care through chronic disease state management and optimizing medication use for ambulatory patients through innovative outpatient clinical services, improving continuity of care by bridging inpatient and outpatient pharmacy services, providing educational instruction to students, leading quality improvement initiatives, and initiating and collaborating in point of care scholarship. 

Additional responsibilities are as follows:

  • Establish a model practice site and provide patient care utilizing collaborative practice protocols
  • Collect therapeutic intervention and outcomes data to track quality indicators for patients managed in pharmacy clinics
  • Assume a leadership role in clinic quality initiatives to meet health-system and national goals
  • Provide experiential instruction to Doctor of Pharmacy students and PGY-1 residents
  • Contribute to the education of medical, pharmacy, and nursing staff
  • Actively engage in scholarly activities with other ambulatory care practitioners, evaluating innovative pharmacist-provided care, ambulatory care therapies, and instructional techniques
  • Take part in service to the profession and community
  • Actively engage in professional association activities
